4. Child's Immunization Record
5. Proof of Income for Prior Year (PK only)
6. Custody/Guardianship Paperwork if Applicable
7. Public Assistance (SSI, TANF, SNAP) (PK only)
Complete online registration for your student
1. Choose Amber Terrace Early Childhood Learning Center for the campus name and upload all the required documents during the registration process.
Attend a district registration event to confirm registration.
